Habitat for Excellence provides a coordinated continuum of community-based services aligned with Ohio Medicaid waiver programs, supporting individuals with developmental disabilities and co-occurring behavioral health needs.

Our services are designed to promote safe, stable, and independent living within the community while reducing reliance on institutional or restrictive care settings. Through structured, person-centered service delivery, we support individuals in achieving functional independence, behavioral stability, and long-term sustainability.

Waiver-Aligned Service Offerings

Provides direct, hands-on assistance with activities of daily living (ADLs) including personal care, homemaking, and routine development.

Homemaker Personal Care (HPC)

Provides direct, hands-on assistance with activities of daily living (ADLs) including personal care, homemaking, and routine development.

Services are delivered in the individual’s home and focus on:

  • Maintaining health and safety
  • Building daily living skills
  • Reinforcing structure and consistency

Non-Medical Transportation (NMT)

Provides access to essential services and community integration through coordinated transportation support.

Services include transportation to:

  • Medical and behavioral health appointments
  • Employment and vocational programs
  • Community-based activities

This service ensures continuity of care and supports consistent participation in daily routines and services.

Semi-Independent Living (SIL)

Provides ongoing, structured support to individuals residing in their own homes or apartments who require supervision, skill development, and routine stabilization.

Services focus on:

  • Skill-building for independent living
  • Behavioral and environmental stability
  • Supervision based on assessed need

SIL services are tailored to the individual’s level of care and are designed to promote long-term independence while ensuring safety.
